Our Story

Healthcare the Way It Was Always Meant to Be

Hometown Primary Care opened in November 2022 with a clear purpose: to practice healthcare differently.


With 16 years of experience in healthcare, including 10 years as a registered nurse and 6 years as a family nurse practitioner, Candace Herrin, APRN, FNP-C, has seen firsthand how powerful care becomes when it is personal, consistent, and rooted in real relationships. She had also seen what happens when those qualities get lost inside a system driven by volume and speed.


From the very beginning, this practice operated on a relationship-first approach. Over time, it became clear that a full direct primary care model in Choctaw was the best way to protect and expand that approach for every patient who walks through the door. The structure evolved. The way we practice care never did.

The People Behind Your Care

Hometown Primary Care is a locally owned, nurse practitioner-led practice. Every person on our team is here because they believe care should be personal, and that patients deserve a provider who truly knows them.

Candace is a board-certified family nurse practitioner with 16 years of clinical experience, including 10 years in Emergency Medicine as an ED RN and Charge Nurse, as well as experience in pulmonology, sleep medicine, OB/GYN, and urgent care before founding Hometown Primary Care in 2022.


She has called Eastern Oklahoma County home for over 30 years. She and her husband, Ray, who works at Tinker Air Force Base, have raised four children in this community and are proud grandparents to one granddaughter. She did not just open a clinic here. She built it for the community she has always been a part of.


Candace is licensed in both Oklahoma and Florida, supporting continuity of care for patients who split their time between states.
