What Is Direct Primary Care?
Direct Primary Care, or DPC, is a healthcare model that removes insurance companies from the equation. Instead of billing your insurance for every visit, you pay Hometown Primary Care a flat monthly fee.
In exchange, you get unlimited office visits, longer appointments, wholesale lab pricing, and direct phone, text, and email access to your provider.
That structure changes how care actually gets delivered. A DPC panel is capped at about 500 patients, which makes 30- to 60-minute visits, same-day scheduling, and real follow-through possible.
DPC fits people who want a primary care relationship that goes
deeper than a once-a-year checkup. Many of our members carry a high-deductible insurance plan or a healthshare plan for hospital coverage and use Hometown Primary Care for everyday care. Others use the membership on its own.

Founder & Family Nurse Practitioner
Hometown Primary Care opened in November 2022 with a clear purpose, to practice healthcare differently. Candace Herrin, APRN, FNP-C, is a board-certified family nurse practitioner serving Choctaw and Eastern Oklahoma County. She founded the clinic after 16 years in healthcare, including 10 years as a registered nurse and 6 years as a Family Nurse Practitioner.
After watching too many patients fall through the cracks of rushed, volume-driven medicine, she built the practice she knew her family and community deserved. The goal was simple: to be the best primary care clinic in Choctaw for people who want a real relationship with their provider.
Candace has lived in Eastern Oklahoma County for over 30 years. Her husband Ray works at Tinker Air Force Base, and together they have raised their 4 children and recently become grandparents to a sweet baby girl. This is not a corporate clinic with a Choctaw zip code.
It is a Choctaw practice, built by a neighbor, for neighbors.

HRT at Hometown Primary Care includes women's HRT and men's HRT. We use bioidentical hormone replacement therapy delivered through creams, patches, injections, and pills, tailored to each patient's specific needs and goals.
Triton Laser Hair Removal
Triton uses three laser wavelengths in a single device, making it safe and effective for all skin tones. Most areas need a series of treatments to reach a significant reduction. We treat the full body, face, and intimate areas at our Choctaw clinic.
EmpowerRF
EmpowerRF is the platform behind two of our most-requested treatments. Morpheus 8 Face and Body uses RF microneedling to firm and smooth skin. Intimate Wellness Rejuvenation supports women through urinary incontinence, vaginal dryness, and pelvic floor concerns. Cherry financing available.
IV Infusions and Injections
Our IV menu includes the Myers cocktail, NAD therapy, immune boost, migraine relief, hangover relief, hydration, and B12 and lipo-mino-mix-c (skinny shot) injections. Each infusion is administered in our clinic by our team. Most appointments take 30 to 45 minutes.
